Help shape the future of Advance Care Planning

End-of-life planning is something that affects us all, but it can be hard to talk about. That’s why, throughout 2024 and 2025, Linkwest has been working with 26 Neighbourhood and Community Resource Centres across Western Australia to support conversations around Advance Care Planning. Now, they’re inviting people from all walks of life to take part in a short survey.

Linkwest is keen to understand the impact of tackling these important topics from a local, community level. The findings will help guide future work and support advocacy for more investment in community-led, place-based initiatives.

The survey takes just 10 minutes to complete and is open to anyone in WA. You don’t need to have any previous knowledge of Advance Care Planning, just a willingness to share your thoughts.

All responses are completely anonymous and confidential. No one will know whether you’ve participated.

By taking part, you’ll help shape how communities across WA are supported to have meaningful conversations about end-of-life planning. Linkwest looks forward to sharing the findings later this year.
