WA Seniors Awards Registration

Advocare will not use photographs, videos or audio (recording/s) of you without your prior written consent. If you are under 18 years of age, we require the consent of a parent/guardian.

We will take reasonable steps to ensure the recording(s) is used solely in accordance with your consent provided below.

What I am consenting to:
Photos, video or audio may be recorded at the event. I give the Department of Communities, Advocare and Lotterywest permission to collect, use and share recordings to promote and report on the WA Seniors Awards.

Privacy:
• We collect your contact details and selected recordings to manage your consent and use the recordings as described in this form. Consent is optional.
• We may share your recordings and identifying details with each other and media channels where required for the purposes in this form or permitted by law.
• Each organisation handles personal information under its privacy policy and legal obligations. You may request access or correction, or make a privacy complaint, using the contact details below or the relevant privacy policy.

Storage, security and retention:
• We will take reasonable steps to protect your information and keep it only while needed for a lawful purpose or as required by law. When no longer required, we will take reasonable steps to destroy it or remove identifying details.

Withdrawal of consent:
• You may withdraw consent at any time by contacting the relevant organisation. Withdrawal applies only to future use and does not affect prior lawful use or sharing. It may not remove material already published, shared or copied, or records that must be kept by law.
• This consent is limited to the purposes in this form and remains valid for three years from the date signed, unless withdrawn earlier.

Payment and rights:
• You will not be paid for the recording or its use. This consent allows the three organisations to use your selected recordings for the stated purposes but does not transfer ownership of your personal information.
