Errol, a 66-year-old man, was referred to Advocare’s care finder program after significant changes in his health, social connections, and emotional wellbeing.
Maintaining mental health is always an important consideration across the lifespan. Following a heart attack five years earlier and subsequent redundancy, Errol experienced severe depression and increasing social isolation.
While he had family living locally, contact with them was limited, and relationships were strained. His mental health challenges, combined with a chronic heart condition, affected his ability to manage daily living tasks, resulting in a home environment that had deteriorated and become unsafe.
After receiving a referral, a care finder team member worked alongside Errol to identify goals focused on improving his safety, wellbeing, and living conditions. They supported him to connect with My Aged Care and the Aged Care Assessment Service, leading to approval for specialised home support funding, Commonwealth Home Support Programme services, and Support at Home assistance.
Building trust and rapport was a key priority throughout our engagement with him. Errol was linked with free telehealth health coaching, domestic assistance, transport, and meal delivery services. These supports significantly improved his home environment and confidence levels.
Following his transition to the Support at Home program, Errol was encouraged to access psychology services to strengthen his mental health. Ongoing coordination of services to meet his complex needs ensured Errol received responsive support when he really needed it, helping him maintain independence, safety, wellbeing and stability over his situation. Connecting him back into his community has made a welcome difference to his quality of life.
